How to add multiple questions to a form page

Each question or Statement you add to your form is considered a form page. Respondents will see one question on a page at a time as they answer your form. However, you also have the option to ask multiple questions on the same page by dragging and dropping questions into a form page or by clicking the + Add content button on a form page to add additional questions.

When you add multiple questions to a form page, your respondents will see something like the image below.

Add multiple questions to a form page

1. Click + Add content and add a compatible question to your form.

The question added will appear as a page on the left side menu.

2. Click + Add content on the form page to add another compatible question to your form.

3. Select a compatible question type to add to your form page. If you select a question type that isn't compatible with a form page, it'll be added as a new page instead. 

You can add as many compatible questions in a form page as you'd like, but we recommend not to add too many as respondents could get overwhelmed on a single page.

4. Click the 3 dots on a question to change the order. You can select to move it up or down.

Alternatively, you can click on a question and drag them around to change the order.

5. Optionally, you can enter a question header to your form page to give respondents context to the questions on the page.

When you click on the header of a form page with multiple questions, you can add a header with Text or you can add a header with a video question. Click Text or Video on the right side panel.

You can also add an image, video, or GIF to your form page header. Click the + button next to Image or video.

Add logic to questions in a form page

You can add logic in the following ways:

  • Add logic to an individual question within a form page
  • Add logic to send respondents to a form page with multiple questions
  • Use answers provided from a form page in logic rules.

1. Click on Workflow and then Branching.

2. Click + Add rule on the question that you want to add logic to.

Form results

Once you’ve collected some responses, go to the Results panel to see your data. Answers to each question in a form page will be shown as standalone answers in the Responses tab.

FAQ

Can I connect to integrations with multiple questions in a page?

Yes. When you connect a form that has multiple questions in a form page to another tool, it works just like any other question type. You can select the form page to be mapped and then map each question within the page to a different field in the integration (if the question type is supported by the integration).

Can I add a Partial Submit Point in a form page?

No, you won’t be able to add a Partial Submit Point in a form page. But, you can add a Partial Submit Point in between form pages.
 

Does Response enrichment work with an email question in a form page?

Yes, you can collect enriched responses with an email question in a form page if it is the first email question of your form.
 

Can I add a score or create an outcome quiz with questions within a form page?

Yes, you can add a score or create an outcome quiz with questions within a form page.
 

What form modes can I use with multiple questions on a form page?

You can use any form mode with multiple questions in a form page. Learn more about form modes here.
 

Can I embed a form in an email if it starts with multiple questions in a form page?

No. Launching your form in an email that starts with multiple questions in a form page is currently not supported.
 

Will responses to multiple questions in a form page show in my response summary report?

Yes, responses to multiple questions in a form page will show in your response summary report. Responses to multiple questions in a form page will also be displayed in your response summary.


What is the difference between adding multiple questions in a form page and Question Groups?

Multiple questions in a form page will show multiple questions at a time on a single page in the form. Similar to the image below.

In the Content panel, multiple questions in a form page will appear with capital letters for each question in the left side menu (e.g. A, B, C, etc.) and you’ll see the questions on one page in the middle editor.

A Question Group, groups questions together under an umbrella. Questions within the Question Group will be shown one question on a page at a time as shown in the GIF below.

In the Content panel, a Question Group appears with the same question number and a lower case letter for each question (e.g. 1a, 1b, 1c, etc.) in the Question Group on the left side menu. You’ll also see one question per page in the middle editor.
