Collect contact details on one page with the Contact Info question type. Contact Info is a preconfigured multi-question page that you can adapt to your needs: add or remove fields, change their order, edit each field, and add other compatible questions.
Important: If you added a Contact Info question to your form before August 27, 2026, your form may be using a legacy Contact Info question type. In this case you’ll need to add a separate, new Contact Info question directly below the old one. This does not replace or convert the old question, which remains in your form until you delete it manually. If the old question contains historical responses that you want to keep, do not delete it. Instead, follow the migration steps below to let respondents use the new question while keeping the old question and its data.
Add a Contact Info question to your form
- Open an existing form or create a new one.
- Click + Add content.
- Select Contact Info.
A multi-question page is added to your form with fields for:
- First name
- Last name
- Phone number
- Company
You can keep the fields you need and customize the page before you publish your form.
Customize the fields on your Contact Info page
Each field on the Contact Info page is an individual question. This means you can manage the fields in the same way as questions on any other multi-question page.
Add a field
To add another field to your Contact Info page:
- Click + Add content on the Contact Info page.
- Choose a compatible question type.
You can add questions such as Short Text, Email, Phone Number, Dropdown, or other question types supported on multi-question pages. The new question is added to the same page.
You can also drag a compatible question from your form into the Contact Info page.
If you select a question type that isn’t compatible with multi-question qages, it’s added as a separate page instead. For the full list of compatible question types, see How to add multiple questions to a form page.
Remove a field
To remove a field from the Contact Info page, drag it outside the page. It becomes a separate question page in your form.
You can also delete the field from your form by clicking the three-dot menu and selecting Delete.
If you remove all but one question from the page, the remaining question becomes a regular single-question page.
Reorder fields
To change the order of fields on the page, drag a field to a new position. You can also click the three-dot menu on a field and select Move up or Move down.
Put related fields together and place the most important fields first to make the page easier for respondents to complete.
Edit a field
Click a field on the Contact Info page to edit it. You can update its question text and use the settings available for that question type.
For example, you can:
- Make a field required or optional.
- Change the default country code for a Phone Number field.
- Customize placeholder text when the question type supports it. See Customize the placeholder text of your questions.
If you make one or more fields required, respondents must answer those fields before they can continue. Optional fields can be skipped.
Use logic with Contact Info fields
Because each field is an individual question, you can use fields on a Contact Info page in logic rules supported by multi-question pages.

Collect and use Contact Info responses
Respondents see all the fields on the Contact Info page together. In your results, each field is saved as a separate answer.
You can map each Contact Info field to separate contact properties in Typeform Contacts as well as to fields in supported integrations. For example, you can map the Email field to an email property and the Phone Number field to a phone property.
If you use Response enrichment, the Email field in a Contact Info page works like an Email question. It can collect enrichment data when it’s the first Email question in your form and Response enrichment is available for your plan. See Response enrichment with Typeform.
Switch a legacy Contact Info question to the updated format
If you added Contact Info to your form before August 27, 2026, your form may contain a legacy Contact Info question type.
You'll need to add a separate Contact Info page directly below the legacy Contact Info question. Adding the new Contact info page does not convert the legacy question which remains in your form until you delete it manually. Deleting the legacy Contact Info question will also delete any responses you’ve previously collected. Do not delete the legacy Contact Info question if you’ve already collected responses that you want to keep. Follow the workaround instructions below.
The legacy question type has more limited field editing and question settings. The updated Contact Info page (as described in this article) gives you separate fields that you can edit, reorder, or remove, and lets you use supported question settings, validation, and logic. If the legacy question has collected responses, keep it in the form until you confirm that its historical data is no longer needed. Use logic to skip respondents over the legacy Contact Info question type to hide it.
Keep historical data when you switch
To let respondents use the new Contact Info page while keeping the legacy Contact Info question and its historical data in your form:
1. Add a new Contact Info page to your form. Click + Add content and select Contact Info.
2. On the question immediately before the legacy Contact Info question, open Workflow > Logic > Branching and add a branching rule that always skips past the old legacy Contact Info question.

3. Leave the legacy Contact Info question in your form. Don’t delete it.
4. Publish the edits to your form.
With this setup, respondents go to the new Contact Info page and don’t see the legacy Contact Info question. The legacy Contact Info question remains in the form so its historical response data stays available in Typeform.
If you don’t need to keep historical responses, you can delete the legacy Contact Info question. For forms that are live and collecting responses, we recommend testing the change on a copy first. Learn more about editing a form that’s already live.
FAQ
Can I add questions that aren’t contact fields?
Yes, as long as the question type is compatible with multi-question pages. This can be useful when you want to collect related information, such as a job title or team name, on the same page.
Can respondents skip some fields?
Yes. Mark the fields you need as required and leave the others optional. Respondents can skip optional fields and continue through the form.
Can I add a Partial Submit Point to a Contact Info page?
No. Partial Submit Points can be added between form pages, but not inside a page containing multiple questions. Learn more about collecting partial responses.
Can I use Contact Info fields in integrations?
Yes. Contact Info fields are sent as individual questions. You can map each field to a different integration property when the question type is supported by that integration.
